Most of these questions bit vague to know how to answer really:
1) How do you believe life began? You mean in the first place? We don't really know. Besides, our definition of life is only based on the way it occured on one planet. We could well find things elsewhere in the universe that blur the definition of what is life. Even some things on Earth (eg: viruses) could be classed as either alive or not, depending on the day of the week.
2) What do you think happens when we die? To what? From whose point of view?
What I thing happens: our bodies rot, and our self-consciousness ceases. But are we our consciousness? Are we just our body?
3) What do you think heaven is? How do you envisage it? Heaven is a state of mind. You can exist in heaven right here and now if you want to.
4) What do you think hell is? How do you envisage it? See heaven.
5) Do you believe in purgatory or an in-between stage between heaven and hell? What do you envisage it to be like? I think most people live in this place for most of their lives.
6) Do you believe in God, what does he mean to you and what do you think he looks like? Mankind has created thousands of gods, I doubt we're likely to stop any time soon. God looks like whatever the society at the time wants and needs. That's why we create them in our image.
7) Do you believe in angels and what do you think they look like? They're just minor deities cooked up for Middle Eastern folk stories, so see Q6
8) What images come to mind when you think of life?
9) What images come to mind when you think of death?
10) What images come to mind when you think of the afterlife?
I'm a pragmatic analytical type, I don't really do "images". I tend to think of concepts in terms of their relationships to other things, as an abstract idea. An afterlife seems like a pretty silly idea though. There's no reason to think that there is one, it's just wishful thinking.
I think Pacman has answered those better than I ever could
