4 months is awkward. Buy something too expensive and you may seem clingy, by something cheaper and you may seem like a cheapskate.

A decent T-shirt is OK. Just help him to enjoy the day, you probably know what he likes so make sure he gets lots of it. The T-shirt will be a bonus.
